Patient-Centered Medical Home (PCMH)

The Center for Living Well is your medical home. We would like it to be the first place you think of for all your healthcare needs including prevention, wellness, acute care, and chronic care.  PCMH offers you personalized care plans, medication review, coaching, advice, and support. The CLW care team will guide your way and coordinate your care as you make your journey to better health.

A PCMH is not a building, a house or a hospital. It is a partnership with your primary care provider; relationship based, with an orientation toward the whole person. PCMH staff work with each patient to develop a personalized care plan that respects each patient’s unique needs, culture, values, and preferences. Your primary care team will help you navigate all of the elements of the complex healthcare system and provide the right care at the right time by connecting you with community specialists and resources when you need them. 

 The Center for Living Well is accredited by the Accreditation Association for Ambulatory Health Care (AAAHC).

Management Team

The Center for Living Well Management Team is dedicated to ensuring that you experience best in class patient satisfaction with each encounter with our team.  The management team consists of the following individuals:


Erica Thomas, Director of Client Operations

Erica received her Bachelor of Science in Health Science Education from the University of Florida in 2002. Prior to joining Premise Health in 2014, Erica led Account Management teams in the Pharmacy Benefit Management space where she aligned pharmacy programs to client’s healthcare needs. When Erica isn’t managing the team at the Center for Living Well, she spends time with her husband and four girls, where of course, visiting Disney is a regular activity.


Dr. Richard Bell, Sr. Executive Medical Director

Dr. Bell received his Medical Degree from The University of Nebraska College of Medicine in June, 1981. Dr. Bell has enjoyed a full and well-rounded Medical Career including experience in Emergency Medicine, Family Practice and as a Medical Director for a Medicare ACO. Dr. Bell joined The Center for Living Well in 2014 as Sr. Executive Medical Director. As a Physician and Healthcare Leader, Dr. Bell and his team are dedicated to providing the test of today’s healthcare services to the patients of Disney’s patient centered medical home. In his free time, Dr. Bell enjoys golfing, spending time with his children and grandchildren and cruising.


Don Jones, Health Center Manager

Don Jones joined the Center for Living Well in 2014 after working in North Georgia where he was the Operations Manager at a multi- site Community Health Practice. Don has over 30 years of medical experience in the Air Force and in civilian practices leading large scale medical operations and managing day to day healthcare clinics. Don enjoys traveling with his wife and visiting his children and grandchildren.


Brandon Alonso, NP, Clinical Services Manager

Brandon Alonso is a Board Certified Adult Nurse Practitioner credentialed through the American Academy of Nurse Practitioners (AANP). Brandon received his Bachelors and Masters of Science degree in Nursing from the University of South Florida. Prior to joining Premise Health, he worked as a Cardiology ARNP with Orlando Health and served our nations veterans at the Tampa VA as an RN and ARNP in the cardiology and spinal cord departments.  Brandon enjoys spending time with his wife and five children, sports, traveling and volunteering at his church.


Michelle Rosario, MA, Clinical Operations Manager

Michelle Rosario is the Clinical Operations Manager at The Center for Living Well. She has been in the healthcare field for over twenty years. She began her professional career as a Hospital Corpsman in the United States Navy. She became a Medical Assistant and continued her career, serving as an MA in rural Primary Care offices. After several years in the healthcare field, she returned to school and obtained a Bachelor of Science degree as a Healthcare Administrator. Michelle was born and raised in East Tennessee. She enjoys spending time with her family and loves to travel.

Tara Marvin, Operations Support Manager

Tara Marvin graduated from the University of Kansas after studying Psychology and Marketing.  She worked for WIRED Magazine and The Washington Post before co-founding the non-profit organization Leadership 4Girls in 2010.  After managing their operations for eight years, Tara joined the Center for Living Well in 2018.  On sunny days, Tara enjoys going to the Disney Water Parks with her family and playing tennis.


Rebecca Olson, Front Office Manager

Becky came to the Center for Living Well in 2016 with over 20 years in front desk operations, scheduling, medical records and Management experience in a large Cardiovascular practice in Illinois. Becky relocated to Orlando to be closer to her family and enjoys spending time with them, going to Disney, the beach and watching Chicago Cubs baseball.
